removing missing icon-app from smart hub

Hi there, on my TV 50hu6900, a have not working properly application. Icon show on quick bar on smart hub, but in main panel on hub - missing. My tv is rooted, and how can i uninstall applicatin, then install again from fs and console. I reset smart hub, but notyhing happend. From Samsung support tell me to make factory default. I make mistake and update to latest firmware (1301), but root stay on my tv. I be afraid to make factory default, and then mai not make root properly. How can uninstall this app that don't show icon?
