Is your model CI+?
if so you need to first run RSA Disabler, without it your TV won`t recognize the "new" firmware on USB stick.
As precaution for CI+ devices you need to disable watchdog in service menue.
but there are some more advanced people here witch will sure help you.

Nope, Fisrt column is not unique key.
You can understand your device if CI or CI+ by FULL modal name. If your model number is like ln55b650
t2p... you have CI+. Or if you have firmware like T-CHL
CIPDEUC you have CI+ too. But If your firmware is T-CHL7DEUC, than your device is CI.
I mean there is more than one option when you buy ln55b650, one is CI and other is CI+.

Test another FAT32 formated USB Key.

Nope, your version is newer than version at Samsung site "1016". So TV refuse to flash older firmware.
Instead you can use ARFixv2 for CI+ application.

Yes, newer than Samsung firmwares at net. You can also modify your firmware.
Enable Telnet by Telnet Enabler,
Enter TV via Telnet,
Capture exe.img from TV
Modify it via SamyGO Firmware Patcher
Encode exe.img by XOR and AES
Put it to v2005's image directory.
Modify exe image Checksum
Mofidy the version number to 2006
Use RSA Disable to disable RSA check
Flash!
I think this series really dangerous even for me.
So wait for Samsung Firmware official firmware upgrade.
